  • Clear, concise communication with management and peers

  • Maintaining full understanding of NHS business processes and work flow
  • Assign and monitor reports as directed by management
  • Monitor census and tasks and assign/ reassign coverage as directed by management
  • Assist in bilingual phone calls as needed
  • Assist in completing any Loss of Time or disability paperwork for participants
  • Educate the participants on their benefits
  • Monitor compliance with the plan of care
  • Complete a discharge survey on hospital discharges within specific parameters defined per procedures
  • Arrange transportation as needed. Complete the transportation request form
  • Arrange PCP and specialist appointments
  • Arrange DME as ordered by the physician
  • Arrange home health services as ordered by the physician
  • Identify , then refer benefit exceptions to management
  • Communicate with vendors to insure appropriate services are available
  • Assist in coordination of individual cases on as needed basis. For example: getting patients into diabetic counseling assist patients with getting doctor appointments etc.
  • Research patient issues and report findings to management
  • Educate patient on programs offered by Culinary
  • Assist patients with community resources
  • Document all issues within patient access database
  • Track all hospital discharge surveys for performance and outcomes
  • Perform other duties as assigned within the scope of responsibilities and requirements of the job 




  • Microsoft Office knowledge
  • Language: Bi- or Multi- lingual skills (written and verbal): one of which must be English (required)


  • Intermediate Microsoft Office
  • Language: Spanish is preferre
  • Excellent written and verbal communication skill
  • Must be detail oriented
  • Positive and persuasive leadership skill
  • Strong Analytical skills
  • Natural advocate committed to social justice
  • Commitment to sharing information and resources






  • Ability to communicate clinical matters to non-clinical individuals
  • Ability to balance empathy with business objectives
  • Manage competing deadlines and multiple projects in a fast-paced environment
  • Perform the essential functions of this job with or without reasonable accommodation



Education: Four year degree or 4 years work experience in the healthcare field

License: MA or LPN Preferred

Certification Required: N/A

Minimum Years’ Experience: Minimum of 2 years’ experience in a healthcare environment

Other Requirements: Bi-Lingual required
