    Responsibilities

    This position assesses, coordinates and partners with health care providers and participants for timely and appropriate delivery of care across service lines. This position reports to the Medical Director while working collaboratively with other Fund leaders, through training, coaching, and joint development of focused health improvement programs. This position may also oversee Health Promoters (lay health outreach staff) within the region.

    • Trains and coaches Health Promoters and other Fund staff as needed on participant clinical needs
    • Collaborates with Health Promoters to assist participants with chronic conditions, complex needs, and/or high risk potential /special needs
    • Assists Health Promoters in creating and implementing individual participant care plans that involve their providers and family /caregivers. It may also include having Health Promoters connect participants to relevant community resources
    • Increases participants’ ability to self–manage and promote shared decision making. This includes coordinating with other Fund programs aimed at providing group support and education (e.g. Better Living) and individual support and education (e.g. Living Better with Diabetes, CERC Cancer Care).
    • Promotes timely access to appropriate care while decreasing emergency room utilization, hospital readmissions and unnecessary lengths of stay
    • Provides knowledge on medication reconciliation
    • Serves as a contact point for clinical information needs of the Region, working closely with the Medical Director
    • Demonstrates initiative in planning/implementing programs based on current and projected future trends in care delivery with the Medical Director and Regional Director
    • Works closely with other Regional Directors, Fund Nurses and Health Promoters to learn from each other’s experiences, support each other, and spread best practices across the Fund
    • Performs other duties as assigned within the scope of responsibilities and requirements of the job

    Qualifications

    Years of Experience and Knowledge

    • 3 - 5 years of direct experience minimum of Case Management, and/or Managed Care
    • 5 - 7 years of team management experience required in a hospital, payer and/or or healthcare environment

    • Experience and working knowledge of managed health care and the health care infrastructure within the Chicago region

    • Clinical knowledge of chronic disease and complex care management in multiple settings.

    Education, Licenses, and Certifications

    • Bachelor's Degree in Nursing, healthcare Management or related field or equivalent work experience required
    • Required: Active State Registered Nurse license in good standing
