    Responsibilities

    The purpose of this temporary position is to identify, recruit and retain high risk participants to chronic condition management programs and/or to conduct one-on-one coaching and care coordination for participants with complex health conditions.

     

    • Recruits participants to, and retains them in, group chronic condition management programs and/or conducts one-on-one coaching and care coordination through individualized and face-to-face contact
    • Meets with participants at their worksite, home, union office and the hospital
    • Works with existing participant leaders to expand participation in these programs
    • Reviews data to determine priority targets for chronic and/or acute health condition program outreach
    • Arranges, coordinates and facilitates educational events, participant leader meetings, chronic condition workshops
    • Documents work and outcomes in CarMa
    • Driving on Fund Business and/or authorized to drive a Fund-owned or leased vehicle
    • Performs other duties as assigned within the scope of responsibilities and requirements of the job
    • Performs Essential Job Functions and Duties with or without reasonable accommodation

    Qualifications

    Years of Experience and Knowledge

    • 2 ~ 3 years of related experience minimum
    • Working knowledge and experience in healthcare, service sector labor unions, community organizing, and/or outreach and education

    Education, Licenses, and Certifications

    • Associate's degree or equivalent work experience required
    • Valid driver’s license, car, auto insurance, or ability to use public transportation

    Skills and Abilities

    • Intermediate level Microsoft Office skills (PowerPoint, Word, Outlook)
    • Beginner level Microsoft Excel skills with demonstrated ability to learn
    • Required fluency (speak and write) in Spanish or other language common to our participants in the region
    • 10% ~ 15% travel
    • Communication skills (written and verbal)
    • Customer service skills
    • Organizational, analytical and follow-up skills
    • Interpersonal and team skills
    • Ability to work with little supervision
