    Responsibilities

    • Assist in bilingual phone calls as needed
    • Assist in completing any Loss of Time or disability paperwork for participants
    • Educate the participants on their benefits
    • Monitor compliance with the plan of care
    • Complete a discharge screen on hospital discharges within specific parameters defined per procedures
    • Arrange transportation as needed. Complete the transportation request form
    • Arrange PCP and specialist appointments
    • Arrange DME as ordered by the physician
    • Arrange home health services as ordered by the physician
    • Must follow all hospital’s policies and procedures
    • Daily driving within the Las Vegas metropolitan area based upon assignment
    • Communicate with vendors to insure appropriate services are available
    • Assist in coordination of individual cases on as needed basis. Example: getting patients into diabetic counseling assist patients with getting doctor appointments etc.
    • Research patient issues and report findings to management
    • Educate patient on programs offered by Culinary
    • Assist patients with community resources
    • Document in multiple EMR’s
    • Track all hospital discharge screens for performance and outcomes
    • Clear, concise communication with management and peers
    • Ability to multi-task
    • Maintaining full understanding of NHS business processes and work flow
    • Assign and monitor reports as directed by management
    • Monitor census and tasks and assign/ reassign coverage as directed by management
    • Driving on Fund business and/or authorized to drive a Fund-owned or –leased vehicle
    • Performs other duties as assigned within the scope of responsibilities and requirements of the job
    • Performs Essential Job Functions and Duties with or without reasonable accommodation

    Qualifications

    Years of Experience and Knowledge

    • Minimum of 2 years’ experience in a healthcare environment
    • Active Nevada Driver’s License without restrictions 

    Education, Licenses, and Certifications

    • Education: Four year degree or 4 years work experience in the healthcare field
    • License: MA or LPN Preferred
    • Certification Required: N/A
    • Valid, unrestricted state driver’s license
    • Personal vehicle registration and insurance

    Skills and Abilities

      • Microsoft Office skills (PowerPoint, Word, Outlook)
      • Microsoft Excel skills
      • Preferred fluency (speak and write) in Spanish
