    Responsibilities

    The purpose of this position is to serve a key role in the management of an efficient relationship with the Fund’s external Pharmacy Benefit Manager (OPTUM) and to play a pivotal and critical clinical role in deploying a Fund-Wide Population Health Strategy

     

    ESSENTIAL JOB FUNCTIONS AND DUTIES

     

    • Manages the development of clinical pharmacy strategies in concert with the VP of Health Center operations to support an effective and evolving Population Health Program designed to impact clinical strategies around the prevention and management of chronic disease states within the CHF population.
    • Manages the day to day tasks and deliverables associated with the CHF/OPTUM relationship ensuring that business objectives are met while delivering a comprehensive pharmacy strategy to ensure and enhance Participant engagement and satisfaction.
    • Evaluate pharmaceutical claims and clinical data to identify opportunities to improve overall performance and cost effectiveness of CHF’s prescription drug program.
    • Develop educational tools to guide Provider prescribing and Participant engagement in managing chronic disease initiatives through the development of a comprehensive population health strategy to include pharmaceutical interventions.
    • Work with Fund Analytics to identify and develop high-value targets among the most prevalent chronic disease states within the CHF Participant base.
    • Work with Fund’s Medical Management division (Nevada Health Solutions) to modify and improve opportunities for engagement and improvement by identifying a targeted patient population prone to acute episodes and exacerbation of their chronic disease.
    • Lead specialty pharmacy assessments of Plan claims to identify variations across sites of service and make recommendations for improvements.
    • Develop measurement tool to assess impact of Chronic Disease Population Health Management through clinical pharmacy interventions.
    • Participate as needed or requested to support PNT and other areas of potential collaboration with UHH’s internal PBM, Hospitality Rx
    • Performs other duties as assigned within the scope of responsibilities and requirements of the job

    Qualifications

    ESSENTIAL QUALIFICATIONS

     

    Years of Experience and Knowledge

    • 7 ~ 10 years of experience minimum
    • 3 ~ 5 years of management experience required
    • Working knowledge and experience in clinical pharmacy and pharmacy benefit management

    Education, Licenses, and Certification

    • Bachelor's degree in Pharmacy or related field or equivalent work experience required
    • Required: Doctor of Pharmacy (PharmD) or BS Pharmacy degree with optional post graduate training or residency
    • Required: Valid Nevada Pharmacist License

    Skills and Abilities

    • Intermediate level Microsoft Office skills (PowerPoint, Word, Outlook)
    • Intermediate level Microsoft Excel skills
    • Communication skills (written and verbal)
    • Organizational, team, deductive reasoning, problem solving and decision making skills
    • Interpret data, do root cause analyses, determine and implement course of action
    • Manage multiple projects and priorities
    • Ability to work collaboratively with all levels of staff
